Student Health Record Requirements
All Chatham University students are required to submit health-related documentation and clearances based on their college, school, or program of study. The university uses two systems to collect, review, and store this information. Please refer to the grid below to determine which system or systems apply.
Students intending to reside in on-campus housing are required to submit the necessary records through both systems.

Med+Proctor
All students living in campus housing, all undergraduate students, and those enrolled in the College of Arts & Sciences (CAS), Falk School of Sustainability & Environment (FSSE), School of Business & Enterprise (FBE), or the College of Health Sciences (CHS) Athletic Training Program are required to submit their health records and immunization information through the Med+Proctor system.
Required Records
- Immunity against Measles, Mumps, and Rubella

- 2 doses MMR vaccine after age 1 or
- Combination of 2 measles vaccines, 2 mumps vaccines, and 1 rubella vaccine or
- Positive immunoglobulin (IgG) antibody blood test to each (proves immunity)
- Immunity against Hepatitis B

- 3 doses Engerix-B, Recombivax HB, Pediarix, Vaxelis, Twinrix or
- 2 doses Heplisav-B (if over 18yo) or
- Positive Hepatitis B surface antibody blood-test
- Immunity against Meningococcal AYWC

- 1 dose Quadrivalent or Pentavalent at or after age 16
- Pennsylvania requires the completed Meningitis AYWC vaccine
- Immunity against Varicella (Chickenpox)

- 2 doses Varicella vaccine after age 1 or
- Birth in the U.S. before 1980 or
- Positive immunoglobulin (IgG) antibody blood test (proves immunity)
- Tuberculosis (TB) screening questionnaire. All students must complete the TB questionnaire. Follow-up Blood IGRA testing may be required if your screening questionnaire is flagged
Other vaccines, including Tdap (Tetanus, Diphtheria, and Pertussis) vaccine, Hepatitis A vaccine, Human Papillomavirus (HPV9), Meningococcal B, Polio, and annual flu and COVID-19 vaccinations, are highly recommended but not required for students within the identified colleges/schools/programs.
Med + Proctor Vaccine Upload Instructions
- Visit secure.medproctor.com and click New Users-Register.
- Register with your Chatham email address.
- Med+Proctor provides immunization certificates to download and give to your PCP to fill out. Submitting immunizations on the certificate is not required; it is there only if you'd like to use it. Waivers are also available for download under the documents section.
Timeline for Providing Records
Chatham requires that students provide their health and vaccination records before the first day of classes. Failure to provide your records may prevent you from enrolling for subsequent semesters. Students whose programs are completely virtual are not required to upload immunization documentation.
Exxat
College of Health Sciences (CHS) programs using Exxat include Occupational Therapy programs, the Physician Assistant Studies program, Counseling Psychology programs, the Physical Therapy program, and all graduate-level Nursing programs.
Required Records, Submission Instructions, and Timelines
Each program sets its own requirements to meet discipline-specific and site-specific clinical education standards. Enrolled students will be automatically registered in Exxat by their respective programs. Students will receive an email link to the Exxat system as part of the detailed instructions provided by their program. Students are expected to comply with all specified timelines and requirements.
*All students planning to live in on-campus housing must submit vaccination records through both Exxat and Med+Proctor Systems.
